Some Insurance companies cover my services.  Verification of coverage and establishment of payment are necessary before I accept co-pays at the time of service.  We will bill for you and reimburse you if payment is made.  Check, cash, HSA and credit cards accepted.  Please kindly give me 24 hrs notice of cancellation before you cancell or you will be charged unfortunately a fee of $70. Payment is due at the time of visit.
